Instructions for Requesting Your High School Transcript
Current High School Students:
Pick up a Current Student Transcript Request form in the high school library or download the form. All transcripts are sent out free of charge.
Graduated Students:
You may download an Alumni Transcript Request form or follow the instructions below. All transcripts are sent out free of charge.

Send your request in writing to:

or fax your request to 920-668-8605 Attn: High School Office.

Include your name, address, phone number and year of graduation. Sign and date your request.

Provide the name and address of the institution where you would like us to mail your transcript.
